What To
Bring:
If you have dental insurance, please make sure to have your
current dental insurance card available for all dental visits. We'll gladly file a claim for the services
provided; however, you are responsible to pay any deductibles or
non-covered charges before you leave. If your policy is one that pays the
patient directly (instead of the dentist), we ask that you pay all
charges in full at the time of service; however, we will file a reimbursement
claim for you.
PLEASE NOTE:
Drs. Mello and Van Lanen do not participate in any dental network plan.
This means that if your policy has both an “in network” and “out of
network” provision, you are reimbursed at the out-of-network rate for
our services. Before coming to your appointment, please check with your
insurance company to make sure you actually have out-of-network benefits
as some plans don't
offer this option.

Secondary
Insurance: Due to the difficulty of receiving
reimbursement from secondary insurers, we no longer file secondary
claims; however, if you
have coverage under more than one dental plan, we will file with your primary
carrier and give you a Superbill to use for your secondary claim. Simply
let us know that you have secondary coverage when you check out and
we'll print the necessary forms before you leave.
If we've filed an insurance claim for
you, you will be billed monthly regardless of the status of claim
payment. It is the patient's responsibility to monitor the claim payment
process to insure timely reimbursement. We're more than happy to refile
a claim; should this be necessary. Simply contact our Insurance
Department for assistance.
If you have questions concerning the
charges associated with your visit, please contact us. Although there
are some treatments we cannot estimate in advance, we will do our best
to give you idea of projected expenses whenever possible.

Payment Plans
As stated previously, Drs. Mello and
Van Lanen ask that all amounts due be paid in full at the time of
service and do not offer monthly payment arrangements through our
Billing Department; however, we recognize that some dental treatments
can be expensive and often occur when it's the least financially
convenient. To assist our patients with payment arrangements, we have
partnered with Care Credit; a
national financial services company. Care Credit offers a wide variety
of monthly payment plans; many with an interest free option.
You can apply for Care Credit
online or obtain applications
from our office. We will be glad to answer any questions you may have
concerning Care Credit and encourage you to take advantage of this
helpful service.
